Addiction Care

Evidence-based treatment for Opioid Use Disorder using Medication Assisted Treatment (Suboxone), wrapped with counseling, case management, peer support, vocational support, and tobacco cessation.

Who it’s for: Anyone struggling with opioid or substance use disorder. Currently accepting new patients.

Primary Care

Person-first primary care for all ages — wellness exams, chronic illness management, immunizations, women’s health, pediatrics, and preventive care. Clinicians see fewer patients than typical practices, so visits aren’t rushed.

Who it’s for: Adults, seniors, children, and families. Currently accepting new patients.

Behavioral & Mental Health Care

Counseling, therapy, and psychiatric medication management, Standalone Services where appropriate — or ntegrated with addiction and primary care. Available statewide across Kentucky via telehealth.

Who it’s for: Anyone experiencing depression, anxiety, co-occurring mental health and substance use conditions, stress, grief, or adjustment challenges.

PrEP for HIV Prevention

PrEP (pre-exposure prophylaxis) is a once-daily pill that is more than 99% effective at preventing HIV. We prescribe Truvada or Descovy after a baseline HIV test and labs — statewide via telehealth in Kentucky.

Who it’s for: HIV-negative Kentuckians at risk — including partners of HIV-positive individuals, anyone with a recent STI, those with inconsistent condom use, or those who share injection equipment.

Telehealth (Statewide Kentucky)

See a Fresh Start clinician from anywhere in Kentucky. Most of our addiction care, Hepatitis C treatment, PrEP, primary care follow-ups, and behavioral health visits can be completed by secure video.

Who it’s for: Any Kentucky resident. Especially helpful for patients without easy access to one of our four clinics, or for follow-up visits between in-person appointments.
