If you have a patient that you’d like to refer to our clinic, please reach out to one of our care coordinators. We would like to have as much patient information as you can provide: demographic information, medical history, social history, and risk factors. If you cannot provide that information—it’s okay. We’ll make sure to do the work to get what is needed.
