One of the most common questions we hear at Fresh Start Health is simple: “How often will I have to come in?” It’s a fair question. Your time matters, and frequent appointments can be hard to fit around work, family, and life in Eastern Kentucky. The honest answer is that visit frequency in medication-assisted treatment (MAT) changes over time—and at Fresh Start, we work to space those visits out as soon as it’s safe to do so.
Why visits are closer together at the start
When you first begin treatment for opioid use disorder, your care team needs to see you more often. Early on, we’re finding the right medication and dose, watching how you respond, and making sure cravings and withdrawal are well controlled. These first weeks are also when support matters most, so more frequent check-ins help us catch problems early and adjust quickly.
During this phase, visits might be weekly or every couple of weeks. That’s normal, and it’s temporary. The goal of this closer contact is to get you stable—not to keep you coming in indefinitely.
As you stabilize, visits can spread out
Once your medication is working, your cravings are under control, and your life is finding its footing, there’s usually no medical reason to keep coming in every week. As patients stabilize, we work toward spacing visits further apart—often to monthly appointments. Monthly visits are enough to renew your prescription, check in on how you’re doing, and keep you moving forward, without disrupting your routine.
This isn’t a shortcut. It’s what good, evidence-based care looks like: matching the intensity of visits to where you actually are in recovery.
How we’re different from other clinics
Here’s something worth knowing: many Kentucky clinics keep patients on weekly visits for the first two months, then every-other-week visits for another three to four months—so it can take close to half a year before you’re allowed monthly appointments. Some group-based programs expect a weekly meeting indefinitely, with no real end in sight. For a stable patient, that’s far more often than the medicine requires. It costs you time, gas, and time away from work and family.
We see it differently. We value our patients’ time. When you’ve done the work and you’re doing well, we don’t make you keep proving it with weekly trips to the clinic. Our job is to support your recovery in a way that fits your life—not to build your life around our clinic schedule.
Telehealth makes spacing out easier
Because Fresh Start offers telehealth across Kentucky, many stabilized patients can handle routine check-ins by secure video instead of driving to one of our four Eastern Kentucky clinics in Ashland, Grayson, Morehead, or Vanceburg. That flexibility is another way we keep treatment from taking over your calendar.
Want even fewer visits? Ask about injectables
If you’d prefer longer stretches between appointments, long-acting injectable medications are an excellent option. Instead of taking a daily film or tablet, you receive a once-monthly injection that steadily controls cravings and withdrawal for the whole month.

- Sublocade — a monthly buprenorphine injection given by your provider, designed for patients who are already stable on Suboxone.
- Brixadi — a flexible buprenorphine injectable available in weekly or monthly formulations, giving your care team more options to match your needs.

For the right patient, injectables like Sublocade and Brixadi mean fewer visits, nothing to take every day, and no bottle of medication to keep track of at home. Many patients find this the most convenient and private way to stay on track.
The bottom line
In medication-assisted treatment, more frequent visits at the start are about getting you stable—not keeping you tied to the clinic. As soon as it’s safe, we work to space visits out, often to monthly, and injectable options can stretch that even further.
